Have you thought about decisions you would want to make if you were unable to make them, and have you included these in your advance healthcare directive?  National Healthcare Decisions Day is this month, and it's a good time to think about making your wishes known to your loved ones.  Here are information and links to your state's forms.  We also have a booklet and digital resource available at the church called Intentional Conclusions.  Designed by Rev. Michelle, it's a place where you can summarize your wishes and collect your thoughts for your family so they would have it in the case of your death.  Printed versions are available at the Book Corner on Sundays, and digital versions and other resources are available for download here.
